      • Patients are evaluated using a PET scanner after intravenoulsy injected with a radioisotope.     • What is an Echocardiogram for?

Type of Services:

  • Cardiac Resynchronization Therapy
    • Cardiac resynchronization is a treatment for heart failure patients with left bundle branch block. It is achieved by implanting a pacemaker or defibrillator with three leads, an atrial, right ventricular and left ventricular lead. The procedure to implant a cardiac resynchronization device is similar to a permanent pacemaker or defibrillator implantation.
  • Coronary Artery Stenting
    • Coronary artery stenting is a procedure used to treat coronary artery stenosis.
    • A coronary stent is a tube-like piece of metal scaffolding which is implanted at the site of narrowing in a coronary artery to widen it. It is delivered to the site of narrowing through a coronary catheter which is inserted through an artery in the groin or wrist. In the factory packaging, the stent is wrapped around a tiny balloon catheter which is kept deflated. The balloon catheter is delivered to the site of narrowing and the balloon is inflated, thus expanding the stent. The stent adheres to the inner wall of the blood vessel. Over time, a layer of cells grow over the stent and cover it so that the stent becomes incorporated in to the artery wall. The stent is left permanently in the artery.
    • After the stenting procedure, the catheter is removed and pressure is exerted over the entry site in the groin or wrist until bleeding has stopped. A pressure bandage is applied, the patient is kept on bed rest for 4 to 6 hours, and his vital signs are monitored. If there are no complications, he may be discharged the next day. He may resume activities as advised by his cardiologist. He has to take two types of blood thinning medication to prevent a clot from forming at the site of the stent. He may stop one of the blood thinning medications only when his cardiologist advises him that it is safe to do so. Under no circumstances can he stop both the blood thinners without informing his cardiologist.
  • Defibrillator Implantation
    • An implantable defibrillator is an electronic device which can sense the heartbeat and detect ventricular tachycardia and ventricular fibrillation, automatically charge up and deliver a high energy shock to revert the heart to a normal rhythm. It is implanted in the same way as a permanent pacemaker. (See TREATMENT: PERMANENT PACEMAKER IMPLANTATION for more information)
    • During the implantation procedure, after the leads have been inserted and connected to the pulse generator, an additional part of the procedure called defibrillation testing may be performed. The purpose of this is to test the system by inducing ventricular fibrillation, waiting for the defibrillator to detect it and automatically charge up and deliver a high energy shock to terminate the arrhythmia. This provides confidence that the system will function when the arrhythmia actually happens. Defibrillation testing is done under deep sedation and with an external defibrillator backup.
    • Two kinds of implantable defibrillators are available
      • Totally subcutaneous ICD – the whole system is under the skin but not inside the heart.
      • Transvenous ICD – the wire component of the system is inside the heart, the battery component is under the skin.
  • Implantable Loop Recorder Insertion
    • An implantable loop recorder is an ECG recorder used for long term ECG monitoring, for as long as three years. Such long term monitoring is necessary for symptoms such as syncope, when other tests have failed to provide an explanation.
    • It is a small device, implanted on the chest wall, underneath the skin, with the ability to record the ECG continuously, detect an arrhythmic event and store it. When interrogated the information can be retrieved and the diagnosis can be made.
  • Permanent Pacemaker Implantation
    • A pacemaker is an electronic device which can sense the heartbeat and put out electrical pulses to stimulate the heart to beat when its rate falls below a certain set rate. It consists of wires (leads) and a battery (pulse generator). Permanent pacemakers are implanted in patients with low heart rates and symptoms of giddiness, loss of consciousness or breathlessness.
    • Permanent pacemaker implantation is a minor operation performed under local anesthesia with or without conscious sedation. The pacemaker is implanted in the upper chest below the collar bone. A small incision is made and the lead is introduced through a vein in the upper chest area. Using X rays, the lead is guided into the heart chamber and fixated in a stable position. One or two leads may be implanted in this way. Single chamber pacemakers have only one lead while dual chamber pacemakers have two leads. The lead/s are connected to the pulse generator at the other end. A small pocket is made under the skin and the lead/s and pulse generator are placed in this pocket. The pocket is closed by stitching up the incision. A sterile dressing is placed over the incision.
    • Pacemaker batteries usually last 5-10 years. When the battery is depleted, the old pulse generator is replaced with a new one. An incision is made over the old one, the lead/s are disconnected from the pulse generator and connected to the new one and the incision is stitched up again.
  • Radiofrequency Catheter Ablation
    • This is a treatment procedure for cardiac arrhythmias such as supraventricular tachycardia, ventricular tachycardia, atrial flutter and atrial fibrillation.
    • An electrophysiological study is first performed to confirm the mechanism and origin of the arrhythmia. (See TESTS: ELECTROPHYSIOLOGY STUDY for more information)
    • An ablation catheter, similar to the catheters used for the EP study, is positioned at the site of origin of the arrhythmia. The ablation catheter is connected externally to a radiofrequency generator. Radiofrequency energy is delivered through the tip of the ablation catheter. This heats up the tissue in contact with the tip and renders the arrhythmia no longer inducible at the end of the procedure.
    • At the end of the procedure, all the catheters are removed and pressure is applied at the entry sites till bleeding has stopped and a bandage is applied. After the procedure, the patient is put on bed rest for 6-8 hours and his vital signs are monitored. The next day, the groin is checked for signs of bleeding and if there are no complications, he is discharged from hospital. After discharge, he may resume normal activities like walking, but strenuous physical exercise is not advised for two weeks in order to allow the heart tissue to heal. He may be required to continue with medication as prescribed by the electrophysiologist.

Type of Services:

  • Electrocardiography (ECG)
    • An Electrocardiogram (ECG) is a diagnostic test that measures the electrical activity of the heart. Every heartbeat starts with an electrical wave that travels through the heart, causing the heart muscles to squeeze and pump blood towards the other parts of the body.
  • Ambulatory ECG Monitoring (Holter)
    • Usually referred to simply as a Holter monitor, it is a machine used to continuously monitor your heart’s rate and rhythm over a period of 24 hours or more. Within this time, electrodes are attached to your chest with a small recording monitor. Advancements in technology have produced Holter monitors that are more lightweight, smaller and more comfortable.
  • Ambulatory BP Monitoring (ABP)
    • Ambulatory Blood Pressure (ABP) Monitoring measures your blood pressure (BP) at regular intervals over a 24-hour period, during which you may go about your regular daily activities.
    • To perform ABP monitoring, you will wear a blood pressure cuff around your upper arm that will be attached to a small digital device that can measure your blood pressure automatically. The cuff and monitoring device is worn over 24 hours.
  • Exercise Treadmill ECG Test (TMX)
    • An Exercise Treadmill ECG test is one of the most common stress tests performed to evaluate the heart’s response to physical stress. The test involves walking on a treadmill at gradually increasing speeds and inclines. This makes your heart pump harder and faster, and may uncover problems within your heart that might not be detected otherwise.
  • Transthoracic Echocardiography (TTE)
    • An echocardiogram is a painless and non-invasive test used to look at the heart and blood vessels. It is a type of ultrasound scan, which uses high-frequency sound waves to take detailed images of the moving heart to evaluate its structure and function.
    • In transthoracic echocardiography, an ultrasound transducer or probe is placed on the chest. This transducer sends sound waves which enter and travel through the chest cavity. When these waves meet the heart and other surrounding organs, it is reflected back. These reflected waves are known as echoes.
    • The echoes will travel back to the same ultrasound transducer where it is received and converted into images of the moving heart.
    • Transthoracic echocardiography is very safe, and it is one of the most commonly used tests in cardiology.
  • Stress Echocardiography
    • An echocardiogram is a non-invasive test to look at the heart and large blood vessels. Echocardiography uses ultrasound waves to take detailed images of the moving heart.
    • Some heart conditions surface only during physical activity. A stress echocardiogram is used to assess the heart function in response to stress. Ultrasound images of the heart are taken as the heart beats harder and faster during exercise (or simulated with injected medications).
    • The images taken during exercise stress are compared to those taken at rest. If any part of the heart contracts abnormally during exercise stress, this may suggest a lack of blood supply to that area.
  • CT Coronary Angiography (CTCA)
    • CT Coronary Angiography is an advanced imaging technique that uses the (Computed Tomography) scanner to obtain detailed images of the heart and coronary arteries.
  • Coronary Calcium Score
    • A coronary calcium score (or sometimes known as Agatston score) utilises a specialised type of x-ray called a CT scan. It measures how much calcified plaque is in your coronary arteries.
  • Nuclear Myocardial Perfusion Imaging
    • A Myocardial Perfusion Imaging (MPI) test, also known as a Myocardial Perfusion Scan or Nuclear Stress Test, is a non-invasive heart imaging test that shows how well blood flows through to your heart muscle and how well your heart muscle pumps blood to the rest of the body.
    • It helps your doctor discern which area of the heart muscle is not getting enough blood flow. The scans are taken when your heart is under exercise stress and when it is at rest to detect any irregularities.
  • Cardiac MRI
    • Also known as Cardiac Magnetic Resonance (CMR), a Cardiac MRI is a type of Cardiac imaging test used to diagnose various heart diseases. It involves the use of a Magnetic Resonance Imaging (MRI) machine. The machine utilises very powerful magnets and radiofrequency pulses to visualise your heart tissues at a high resolution — allowing the cardiologist to evaluate the heart’s anatomy and function and to detect Cardiac diseases.
    • Some Cardiac MRI tests require the use of a gadolinium-based contrast dye injected into the bloodstream. These are used to accurately localise areas in the heart muscle which are damaged.
  • Coronary Angiography
    • A coronary angiogram is a procedure that looks at the coronary arteries in your heart to detect narrowing or blockages. The coronary arteries are the blood vessels that supply blood to the heart muscle. During a coronary angiogram, a small tube (catheter) is inserted through a large artery in the wrist or the groin, and is advanced to the heart and placed at the openings of the coronary arteries. To see the coronary arteries, a special contrast dye is injected through the catheter and into the blood vessels. Using live X-ray, pictures are taken to detect the location and severity of the blockages. This will help your doctor plan your treatment.
  • Coronary Angioplasty and Stenting
    • Coronary Angioplasty (Percutaneous Coronary Intervention) is a minimally invasive procedure that involves inserting a small balloon catheter into a narrowed or blocked artery to open up the blocked artery. This helps to improve the blood flow to the heart muscles.
    • A small wire mesh tube, known as the stent, is often implanted during an Angioplasty which props the artery open and reduces the chances of the artery narrowing again. Most modern stents are coated with medication which is released gradually to prevent narrowing within the stent and to keep the artery open.
    • Coronary Angioplasty is performed to improve symptoms of narrowed or blocked arteries such as chest pain (angina) and shortness of breath. Coronary Angioplasty is also performed during a heart attack to immediately open an acutely blocked artery and lessen the damage to the heart. Performed emergently during a heart attack, Angioplasty is potentially life-saving.
  • Electrophysiology (EP) Study
    • An Electrophysiological (EP) Study helps assess the heart’s electrical system and to detect any abnormalities in its conduction system that could be causing heart rhythm disorders. It is commonly used to diagnose abnormal heartbeats or arrhythmias and involves the insertion of catheters or wire electrodes to measure the heart’s electrical activity. 
    • An EP study is carried out by cardiologists who specialise in heart rhythm disorders and helps them map the pathways of complex arrhythmias to determine the best course of action when a catheter ablation is done.
  • Pacemakers
    • Pacemakers are a type of Cardiovascular Implantable Electronic Device (CIED) used to treat arrhythmias, also known as heart rhythm disorders. This is done by keeping your heart beating steadily using electrical impulses sent directly to the lower chambers of the heart. The electric shock helps correct any irregularities or abnormalities in the heartbeat.

Prior to joining Cardiac Centre International, Dr Lee was a senior consultant and head of the Department of Cardiology at Khoo Teck Puat Hospital (KTPH). He was also the director of the Cardiovascular Centre, the lead in pacing and cardiac electrophysiology, and the former director of the Coronary Care Unit (CCU) at KTPH. In addition, he was a visiting consultant to the National Heart Centre and Tan Tock Seng Hospital, and an Adjunct Assistant Professor at the Yong Loo Lin School of Medicine and Clinical Teacher at the Lee Kong Chian School of Medicine.
  • With his diverse background, Dr Lee Chee Wan is favorably positioned to provide comprehensive diagnostic and therapeutic care to local and international patients.
